1. Proof of accreditation of the Laboratory as indicated in Appendix 1
2. Proof of Security, as per Appendix 2
2. Mandatory Requirements, as per Appendix 4
3. Completed Rated Requirements, as per Appendix 5
4. Completed Supplier Diversity, Inclusion and Sustainability Programs table, as per Attachment 1
4. Completed and signed Form of Proposal, as per Appendix 7
5. A completed NEQ Table, as per Appendix 10
6. For Respondents bidding on RCMP sites, a completed Employees Cleared at RCMP Buildings Form, as per Appendix 11
